Submit a freedom of information request
You can use this service to send a freedom of information request to the NHSBSA.
Before you start
Check our publication scheme and previous requests and responses to see if we've already published the information you are looking for.
As part of the request, we will ask you for:
- your contact details
- what your request is related to
- the date range that you need the information to cover, if appropriate
If you need to send us supporting information, you will be able to email it to us along with your reference number, when your request is complete.
